Providing Comfort and Care for Families
Throughout the state of Colorado, Home Health agencies provide an invaluable service in helping seniors age in place through medical and non-medical caregiving. At Hallmark Homecare, they are changing the process to help seniors and their families feel more in control and more at ease while safely staying at home.
Meeting with the client to determine the level of care and the services needed, Hallmark Homecare realizes that every client has individualized needs and is able to match their needs to a caregiver and their skills. Once a schedule is set, caregivers will provide the care that is needed. This model enables ongoing consistency so the client becomes familiar with the same caregiver.
With Hallmark Homecare, you receive an assessment and consultation with a care coordinator as well as a meet and greet with two different caregivers. Once you find a caregiver that you are comfortable with, they will be your service provider throughout your care plan. Each caregiver, medical or non-medical is fully vetted through a rigorous screening process. Care coordinators are also available to help whenever needed throughout the process.
