Maybe you participated in rehabilitation before, but have lost some of your gains in mobility and completing everyday tasks are becoming more difficult. Maybe those frequent trips to the Doctor’s office or Emergency Department are starting to add up.

Many conditions qualify
Many conditions are accepted by HealthSouth’s home referral program for admission, including general debilitation with limited mobility and/or deficits in self-care activities, such as bathing, dressing, or eating. Other possible diagnosis include Parkinson’s disease, Multiple Sclerosis, history of a stroke, brain injury, spinal cord injury, amputation, cardiac surgery, neurological disorders, orthopedic surgery, rheumatoid disease, and a back, pelvis or lower extremity fracture.
Meeting admission criteria
Before anyone can qualify for a referral to HealthSouth, there are a few criteria that must be met:
- A clearly established rehabilitation diagnosis and goals
- Patient’s needs or co-morbidities require a physician for monitoring
- Expectation of a discharge home after an inpatient acute rehabilitation stay
- Insurance approval of admission
Patients are not required to have had a three-day acute hospital stay. So, if you meet all of the above criteria, a home admission is certainly within reach.
Understanding the referral process
To begin the process, a rehabilitation liaison evaluates a patient at home via medical record review and patient/family interviews. Next, the HealthSouth medical director reviews the evaluation and determines if the patient is appropriate for admission.
Once accepted into HealthSouth’s inpatient acute rehabilitation program, a team of specialized therapists, nurses and rehabilitation physicians create a therapy plan that focuses on helping you achieve your goals. This includes at least three hours of physical, occupational and/or speech therapies five days a week, and daily physician visits. Patients receive attention with 24-hour nursing oversight. Our goal is for all of our patients to progress in the rehabilitation process so they can regain their independence and be successful living in the comfort of their own home. The average length of stay in our acute rehabilitation hospital is 12 days.

HealthSouth Rehabilitation Hospital of Denver is a brand new inpatient rehabilitation hospital that opened May 15, 2013 in Littleton. HealthSouth offers a full continuum of rehabilitative programs and services offered for individuals who have suffered a major accident or illness, including trauma, stroke, head injury, spinal cord injury, amputation, arthritis, chronic pain, neuromuscular and pulmonary diseases. HealthSouth is one of only two freestanding inpatient rehabilitation hospitals in the Denver metro area.
